The Tale of the Heart Condition: Part II (and a half)

I have a heart condition.  Or, better yet, I had a heart condition.  I always get confused on whether it is supposed to be referred to as past tense.  Am I cured?  Is it still something I need to mention on those doctor’s forms that ask about present health conditions?  I’m not clear on it, but I know for certain that two years ago I had my second heart procedure, and I have been symptom free since then.  My first heart procedure took place nine months before that, but wasn’t successful.  My second procedure was in March of 2010.  They were a bit more thorough the second time around, thank goodness!

So for those of you who have had a condition/syndrome of some sort, but have had something done to “take care” of it, do you consider it in the past tense?  When you are asked on forms if you have any condition of any kind, would you mention it?  What about for life insurance purposes?  I know for a fact that when we got our life insurance, it was before my procedures, and I was considered at “higher risk”, so my premium was much higher.  Could it be in my best interest to get reevaluated?  These are all the things I wonder . . .
